2025 was a turbulent year in medicine and health research globally.1 Attacks on equity, diversity, and inclusion (EDI) policies initiated by the administration of US President Donald Trump; a wave of terminations and resignations at US agencies and the National Institutes of Health; and withdrawal of the USA from WHO all led to repercussions felt by the research and academic publishing communities far beyond the US border. And continued efforts to address research integrity issues, fuelled partly by paper mill activities and inappropriate creation of AI-generated content, resulted in a series of retractions across multiple journals worldwide.
