o Copy of driver’s license, ID card, or other acceptable proof of age - Proof of income:
o Form 540, SS benefit, SSI benefit, General Relief, CalWORKS/AFDC; Notarized letter - Proof of disability:
o A recent certification (last 2 years) signed by a licensed physician - LADWP bill, Gas bill and Telephone bill
