Dutch Public Health Reform: A Shift in Strategy

The Netherlands is reassessing its approach to public health amid concerns about declining health outcomes and widening socio-economic disparities.State Secretary for Youth,Prevention and Sport,Vincent Karremans,outlines a new vision,prioritizing implementation and collaboration over legally mandated health goals.

The Netherlands Grapples with Public Health challenges

Once a leader in European life expectancy, the Netherlands has seen its position slip, now residing in the middle ranks. This decline, coupled with a growing socio-economic health gap and an increase in chronic diseases, has prompted serious concerns about the efficacy of the current public health system.the Council for Public Health & Society (RVS) has previously called for a comprehensive “Delta Plan for public Health” to address these issues.

In their 2023 report, on Our Health, the RVS advocated for legally binding health objectives across all levels of goverment. They also proposed the creation of a public health commissioner to facilitate transparent cooperation and citizen involvement, ensuring that public health policy remains insulated from short-term political cycles. However,the government is taking a different path.

Karremans’ Vision: Implementation and Collaboration Take Center Stage

Instead of embracing the RVS’s recommendation for a government commissioner, State Secretary Karremans emphasizes the need for enhanced implementation and collaborative efforts. He articulated his strategy in a recent letter to the Dutch Parliament, outlining four key areas of focus for strengthening public health. These include working with stakeholders to develop a shared vision for the future of public health policy.

New Consultation structures: Prioritizing Shared Governance

Rejecting the notion of a top-down approach, Karremans is establishing two new consultation structures:

  • Administrative Consultation public Health: This forum will bring together municipalities and other relevant parties to collaboratively determine the direction of public health initiatives, working in tandem with the government.
  • ‘Healthy Conscience’ Guidance Group: Composed of scientists and field experts, this group will monitor long-term developments and provide guidance on public health strategies.

These structures aim to foster a sense of shared obligation and ensure that public health policy is informed by a diverse range of perspectives.

Ensuring Equity: A Uniform Basic package for Municipalities

A key component of Karremans’ plan is the legal establishment of a uniform basic task package for municipalities in the realm of public health. This initiative seeks to address existing inequalities between municipalities and regional health services (GGDs) by guaranteeing a consistent level of service across the country.This package will encompass services already receiving structural funding, such as sexual health programs, initiatives to address health disparities, and preventative programs like “Promising Start” and fall prevention programs for the elderly. According to recent data from the National Institute for Public Health and the environment (RIVM), disparities in access to preventative care contribute significantly to health inequalities across different regions of the Netherlands.

Hesitation on legally Mandated Health goals

While exploring the possibility of legally codifying health goals, karremans expresses reservations. He argues that setting goals implies that health can be made, while ther are many factors of influence. He believes that legally binding targets may overlook individual responsibility and undermine the principle of shared accountability. Furthermore, he raises concerns about the potential legal and financial implications of such a move, indicating a need for further examination.

Centralization vs. Decentralization: A Balancing Act

Karremans acknowledges the importance of clearly defining the roles and responsibilities of decentralized and centralized authorities. He intends to explore the potential for centralizing certain tasks to improve efficiency and effectiveness. A progress report outlining these considerations is expected in early 2026.
