Current treatments for Parkinson’s disease (PD) primarily focus on slowing the disease’s progression. Early diagnosis is crucial for optimizing patient care, as the neurological condition worsens over time. Conventional diagnostic methods,such as clinical rating scales and neural imaging,can be subjective and expensive. However, a recent study published in ACS’ Analytical Chemistry details the development of a novel system that offers an inexpensive way to screen for PD by analyzing odors present in a person’s earwax.

Previous studies have indicated that changes in sebum, an oily substance secreted by the skin, could be indicative of PD. Sebum from individuals with PD may possess a distinct odor due to alterations in volatile organic compounds (VOCs) caused by disease progression, including neurodegeneration, systemic inflammation, and oxidative stress. However, environmental factors can alter the composition of sebum on the skin, making it an unreliable testing medium. Researchers, including Hao Dong, Danhua Zhu and colleagues, chose to focus on ear wax, which primarily consists of sebum and is easily accessible, as it is indeed shielded from external elements within the ear canal.

The researchers collected ear wax samples from 209 human subjects, with 108 having a PD diagnosis, to identify potential VOCs associated with PD. Gas chromatography and mass spectrometry were used to analyze the samples. The study revealed that four VOCs found in the ear wax of individuals with PD differed significantly from those without the condition. The researchers identified ethylbenzene, 4-ethyltoluene, pentanal, and 2-pentadecyl-1,3-dioxolane as potential biomarkers for PD.

The team then trained an artificial intelligence olfactory (AIO) system using the ear wax VOC data. the resulting AIO-based screening model accurately categorized ear wax samples from individuals with and without PD with 94% accuracy.The researchers suggest that the AIO system could serve as a frist-line screening tool for early PD detection, potentially leading to earlier medical intervention and improved patient care.

Future Research Needed

“This method is a small-scale single-center experiment in China,” says Dong. “The next step is to conduct further research at diffrent stages of the disease, in multiple research centers and among multiple ethnic groups, in order to determine whether this method has greater practical application value.”
