Almost every year 120,000 people are hit by stroke. A huge number, which tells of a complex reality: a sudden medical emergency, an enormous social impact and, above all, a large share of cases that could be avoided. It underlines it French Court of Auditors in a report published on October 29, 2025on the occasion of World Stroke Day. The institution puts a key word at the center of its analysis: hypertension. It is the most common risk factor, yet it often remains undiagnosed and untreated. The recurring phrase among experts is clear: “who controls the pressure, controls the risk”. The neurologist also reiterated this Pierre Amarencohighlighting that only half of people with hypertension know they have it, and only a portion of these follow regular treatment. This data alone shows the room for possible improvement.
Stroke as a national emergency: early diagnosis, rehabilitation and territory are the critical points that emerged from the official study
The report traces the path of the patient affected by strokeevaluating immediate assistance, hospitalization, rehabilitation and return to daily life. Over the last twenty years the country has built a dedicated system with specialized units (UNV), emergency protocols and hospital networks. A work that has borne fruit: more patients arrive quickly at the appropriate centers and acute care has improved. Yet critical issues emerge. The first concerns the prevention: too generic, not very personalized, often distant from subjects with high risk. Then there’s the health geography: Some regions lack staff and facilities, creating inequalities in treatment. Another problem reported: the insufficient follow-up after discharge. Not everyone receives adequate rehabilitation, many are not monitored over time and this increases the risk of relapse, which remains high in the months following the event. The consequences are severe: motor difficulties, impaired speech, chronic fatigue, invisible cognitive dysfunctions that change life. Doctors remind us that every minute counts. Recognize the signs — difficulty speaking, facial asymmetry, sudden loss of strength — and calling for help immediately remains crucial.

Within this scenario, the Court of Auditors suggests a change of pace. It proposes a more incisive national plan with targeted strategies, a strengthened territorial network and greater use of technologies such as tele-strokedesigned to support hospitals far from specialized centers. Simple idea: if the neurologist is not on site, he connects via video and guides immediate diagnosis and treatment. The report then cites the need for coordination between general practitioners, specialists and rehabilitation services, because continuity of care reduces disability and mortality. In its conclusions, the Court recalls a sobering fact: the average age of patients is falling. More and more cases are affecting people under 55. And the epidemiological projections speak of a possibility +35% stroke by 2035 if the strategies do not change.
Hypertension under observation: why it remains the biggest risk factor and the first opportunity for real prevention according to experts
The heart of the document is thearterial hypertension. It is defined as “the greatest weakness of prevention”. In France, as in many European countries, it screening it is discontinuous and patient management is not always structured. According to specialists, values beyond 120/70 mmHg they deserve attention and more 130/80 mmHg require treatment in subjects with cardiovascular risk. The factors that increase pressure are known: excessive salt, alcohol, overweight, little physical activity, agechronic diseases. The Court recalls that controlling hypertension is one of the most effective actions to avoid a stroke, with a potential risk reduction of up to80% with correct habits and adequate care. The message is direct: we need a dedicated national strategy, widespread screening, information campaigns and regular contact with the doctor. Many cases can be avoided, and the scientific evidence is clear. The report also calls for improved communication on warning signson emergency procedures — in France it is called the 15 — and on the importance of timeliness.
The economic issue is not secondary. More strokes mean more healthcare costs, continuous care and burden on families. While improving prevention means reducing hospitalizations, disabilities and long-term expenses. The Court defines the current situation as “not sustainable” if the trend does not change. In summary: early diagnoses, constant management, health education and blood pressure monitoring. Those who control their blood pressure today protect their brain tomorrow. A health, social and cultural challenge that involves institutions and citizens.
