Rectum Carcinoma: Latest Guidelines & Updates

In this article, the clinically relevant new aspects of the updated German, Austrian and Swiss Oncopedia guidelines and the S3 guideline for the treatment of locally advanced rectal cancer were summarized. The most important aspects include (i) the use of total neoadjuvant therapy (TNT) in rectal carcinomas with high -risk features, (II) treatment with neoadjuvant chemotherapy in patients with low risk for local recurrence, (III) The immunotherapy with dostarlimab in patients with high grade Microsatellite-stable/Mismatch-Repair deficient (MSI-Hoch/DMMR-) rectal carcinoma and (IV) the implementation of organic treatment concepts. The availability of several evidence -based treatment options requires intensive discussions within the multidisciplinary team and a comprehensive information of patients about the treatment goals, options and risks of the individual treatment approaches.
