Medication Therapy Management (MTM) and Alzheimer’s Disease (AD)
The World Health Organization (WHO) has launched a global initiative aimed at improving the lives of individuals living with dementia by 2025. One of the key targets of this plan is to ensure that 75% of countries provide support and training programs for families of dementia patients.
Another critical aspect of the WHO’s goals is the involvement of multidisciplinary teams. Pharmacists play a pivotal role in these efforts. They offer education on pharmacotherapy options for AD, lifestyle modifications, and drug interaction assessments. By working closely with patients, families, caregivers, and healthcare professionals, pharmacists can significantly enhance the quality of care for dementia patients.

Impact of MTM Programs on Potentially Inappropriate Medication (PIM) Use
A recent systematic review assessed the impact of MTM programs on PIM use in older adults. The study included seven studies conducted in various settings, including tertiary medical centers, outpatient clinics, community pharmacies, and telehealth providers.
Three of these studies specifically focused on patients with AD or dementia. The review found that MTM programs reduced PIM use in five out of the seven studies. One of the significant findings was that MTM services increased the likelihood of discontinuing inappropriate anticholinergic medications. These results underscore the critical role pharmacists play in improving medication management for dementia patients.
The Care Ecosystem Model: A Comprehensive Approach
A randomized controlled trial evaluated the effectiveness of dementia care through telephone and internet-based interventions using the Care Ecosystem model. The study, conducted over a year, involved patients with dementia (PWD) and their caregivers.
Participants were divided into two groups: the Care Ecosystem group, which received consultations from care team navigators and specialized providers including pharmacists, advanced practice nurses, and social workers; and the usual care group, which received contact information for organizations and quarterly newsletters.
Pharmacists in the Care Ecosystem group reviewed medication lists and communicated recommendations to physicians. They also discussed medication changes with caregivers. The results showed that the Care Ecosystem model improved the quality of life for PWD and reduced emergency department visits and caregiver burden compared to usual care.
The Role of Pharmacists in Dementia Care
Pharmacists are essential in multimodal approaches to dementia care. They offer a wide range of services, from educational programs to drug interaction evaluations. By tailoring medication regimens and addressing patient-specific needs, pharmacists can significantly enhance outcomes for dementia patients and their families.
The global action plan by the WHO emphasizes the importance of comprehensive care for dementia patients. The inclusion of pharmacists in multidisciplinary teams reflects the recognition of their critical role in improving patient care and quality of life.
Conclusion
Medical professionals, particularly pharmacists, are instrumental in the management of Alzheimer’s disease. Their involvement in MTM programs and comprehensive care models, such as the Care Ecosystem, demonstrates a commitment to enhancing patient outcomes and reducing healthcare burden.
As the prevalence of dementia continues to rise, the collaborative efforts of healthcare professionals will be crucial in providing effective care. By focusing on medication management and education, pharmacists can make a significant impact in improving the lives of individuals with dementia and their support systems.
Whether you’re a healthcare provider, a family member, or simply interested in learning more about dementia care, the role of pharmacists in MTM and comprehensive care models is worth exploring further.
